The framing effect is choosing differently between identical facts depending on whether they're described as gains or as losses.

There is no neutral frame. Every description picks a side of the reference point.
What it really means
The 1981 experiment gave people an outbreak, 600 lives at stake, and two programmes to choose between. Framed one way: Programme A saves 200 people; Programme B has a one-third chance of saving all 600 and a two-thirds chance of saving nobody. Most people — 72% — took the certain 200.
Then the same choice, reworded. Programme A means 400 people die; Programme B has a one-third chance nobody dies and a two-thirds chance all 600 die. Now most people took the gamble.
The two versions are arithmetically identical. Saving 200 of 600 is 400 dying. What changed is where the reference point landed: in the first framing you're being handed lives, and people take a sure gain; in the second you're losing them, and people will gamble to avoid a certain loss.
The uncomfortable implication is that there is no neutral frame. Every way of stating this puts you on one side of the reference point or the other, which means whoever writes the sentence has already made part of the decision. That's why the serious recommendation in medical consent research isn't 'phrase it fairly' — nobody can. It's to state it both ways and let the gap between your two reactions be visible.

- Identical. In the classic study, shoppers rated beef labelled '75% lean' as leaner and better-tasting than the same beef labelled '25% fat' — though the effect shrank once they actually tasted it. The gain frame does the work, which is why packaging usually reaches for it, even though dairy happily labels fat percentage the other way.
- Why do surgeons say '90% survival' instead of '10% mortality'?
- Because patients consent far more readily to the survival frame, even though the numbers are the same. This is why informed-consent research recommends presenting both — there is no neutral phrasing to fall back on, so the fix is to hear it both ways.
